Membership Pause Form

THIS IS NOT A MEMBERSHIP CANCELLATION REQUEST.

As stated in your membership agreement, you have the option to put your automatically recurring monthly membership on hold for any reason once per calendar year for up to 30 days. Any additional pauses during the same calendar year will be subject to a $50/month charge.

You must fill this out 7 days before the 1st of the month you wish to pause
